- change in your address

- request for Certificates of Insurance
- should an incident, that does or may give rise to a claim, occur between the completion of the CAP Application Forms (Insurance Application and Supplementary Insurance Application) and the requested 'effective date of coverage', immediately contact us. Failure to do so, may void your CAP policy.
